Wellness Coaching Program

HWI’s Wellness Coaching Program takes a holistic, high-touch approach to lifestyle modification, focusing on lowering the participant’s risk for illness, helping the individual achieve and maintain wellness over the long-term. HWI Wellness Coaches serve as a guide, helping to educate and engage the individual so that they make better choices and take the necessary steps to improve their health, with a focus on lifestyle driven health topics such as:

  • Smoking
  • Stress
  • Weight management
  • Nutrition
  • Physical activity

Based on the available data – Personal Health Assessment and biometric screening results and/or the member’s own stated health improvement goals – the Wellness Coach will work with the participant to help them define and achieve their wellness goals. The focus throughout is on a personalized plan for sustainable health improvement.

 

The Wellness Coach will assist the participant in addressing multiple health risks – maintaining the one-on-one collaborative coaching relationship over time as the participant progresses from their primary risks to lower priority risks. The Wellness Coaching Program is also designed to seamlessly integrate with other resources available through the employer's benefit package, such as disease management or employee assistance programs (EAP), to ensure that the individual is supported across the entire health management continuum.

 

HWI uses a customized Coaching platform to track participant information such as health data, goals, and activities. The platform supports comprehensive reporting on participation, graduation rates, and behavior change focus areas, among other data. The platform also supports the scheduling of Coaching appointments, the distribution of health education materials, and progress tracking.
