Personal Health Assessment (PHA)

To develop a complete picture of the health risks facing a worksite population, HWI deploys its proprietary Personal Health Assessment (PHA) tool. The Personal Health Assessment is available online and in print to meet the needs of diverse audiences, and takes roughly 15-20 minutes to complete. Each Personal Health Assessment participant will respond to a series of questions about their current health, medical history, beliefs and attitudes, and personal habits related to behaviors such as exercise, diet, automobile safety, and tobacco or alcohol use.

 

These questionnaire responses can also be complemented by biometric screening data for such vital health indicators as blood pressure, total and HDL cholesterol, and blood glucose. Each individual who has access to this data through screenings available at an HWI Wellness Clinic will have the opportunity to enter this data in their Personal Health Assessment questionnaire. The inclusion of biometric data ensures a comprehensive analysis of the individual’s overall health by enhancing the self-reported data of the questionnaire responses with clinically defined markers for long-term health risk.

 

Integration with Plan Resources

The Personal Health Assessment is engineered to function as a seamless component of a health plan's technology solutions to support member health. Assessment data gathered from subscribers and their spouses or adult dependents can be fed directly into the plan's data warehouse, care management platform, or predictive modeling systems.

 

The online Personal Health Assessment can also be customized to support single sign-on (SSO), allowing plan members to reach the PHA and the resources of HWI's Online Wellness Portal by simply logging into the health plan's Web site. In an SSO implementation, HWI can also make direct access to the online Personal Health Assessment available (outside of SSO) for non-member employees of the health plan's customers. This option provides additional value on behalf of the sponsoring health plan by enabling the employer to assess and manage the health of their total population.

 

Online and Print Options

The online Personal Health Assessment allows for efficient, cost-effective data collection and report creation by eliminating the need for mailing and processing paper forms. When the participant submits their completed Personal Health Assessment responses, they will be connected immediately and seamlessly from the final page of the PHA questionnaire to their home page in HWI’s Online Wellness Portal.

 

Participants in the print Personal Health Assessment will receive a comprehensive Personal Health Risk Profile by mail at their home address within a few weeks of submitting their completed PHA questionnaire. The profile will detail the respondent’s health risks and provide recommendations to guide them in the improvement of their health.

 

Employer Reporting: Health Management Profile

Once the Personal Health Assessment campaign and any potential Wellness Clinics have been completed, a report of the aggregate PHA data known as a Health Management Profile will be generated for the employer’s wellness team and management stakeholders, to help the organization develop a long-term health promotion strategy that will address the aggregate health risks of their members.

 

The observations and recommendations of the Health Management Profile are supported not only by health risk data, but also by an analysis of the population’s mindset and readiness to change. For the purposes of participant confidentiality, HWI requires a minimum of 50 Personal Health Assessment participants to generate any aggregate reports.
