A targeted, comprehensive wellness strategy begins with an assessment of population health risks, medical care and productivity costs, available communication channels, and other factors.
HWI's trained Health Management Consultants work with each health plan client to develop a customized profile of the plan's employer groups and membership, based on claims data, questionnaire responses, diagnostic screenings, onsite analyses, and other available data. These assessment results can then be used to design a worksite wellness initiative that addresses health risks for the population as a whole, while meeting the specific needs of each employee.
Throughout, the Health Management Consultant works with health plan stakeholders to ensure that any worksite wellness initiatives align with plan business objectives and integrate with the plan's resources for condition and care management, supporting the individual across the entire continuum of care.
