The Health Interest Survey is an employee questionnaire designed to gather information about member health interests and the level of support members might receive to help them reach their health-related goals.

The data collected will be analyzed by HWI and used to determine employee interest in risk prevention and wellness initiatives, along with preferences related to factors such as class length and format. The Health Management Consultant will then tailor a wellness program plan based on the results, discussions with employer stakeholders, and a review of data from the Personal Health Assessment or Organizational Health Audit (as available).
